After the workshop, complete the online evaluation form to receive CE credit, and indicate which certificate you are seeking. FCP will e-mail CE certificates the first week in November unless you request a paper copy via mail, in which case your certificate will be mailed. Follow the instructions on the form.

For Mandated Reporting credit, please follow the link in the Mandated Reporting Credit box. Complete the form and submit. This information will be sent to NOVA, which will upload the Mandating Reporting credit to the state licensing board.

Submit your Evaluation and Mandated Reporting Credit Request by October 21 at 5 p.m. (links expire at that time).
