After the workshop, complete the online evaluation form to receive CE credit, and indicate which certificate you are seeking. FCP will e-mail CE certificates by the end of April unless you request a paper copy via mail, in which case your certificate will be mailed. Follow the instructions on the form.
Submit your Evaluation by Friday, April 7, at 5 p.m. (link expires at that time).
