After the workshop, complete the online evaluation form. For those seeking CE credit, indicate which certificate you are seeking. FCP will e-mail CE certificates unless you request a paper copy via mail. Follow the instructions on the form. Pennsylvania educators seeking Act 48 credit must complete the Act 48 Assessment form. FCP will upload your credits directly into the Act 48 system upon completion and send a confirmation e-mail once the credits have been uploaded. You must have your PP ID# for the assessment form. Submit your Evaluation and Act 48 Credit Request by April 8 at 5 p.m. (links expire at that time).
